Refractive Surgery

Beginning from the eras of PRK, LASIK then shifting to Contoura Lasik, Dr. Piyush Unadkat has always believed to evolve of advance with times to provide the best in class cutting edge technology to his patients. For patients having very high refractive errors and less corneal thickness where flap based surgeries is not possible he also provides advanced phakic lens surgery.

Staying true to our principle of providing our valued patients with the most advanced and superior medical technology, SEHLC is proud to Announce that we are the First Eye Hospital in Saurashtra to bring the next generation of laser vision correction solution, the SILK (ELITA Femtosecond Laser )to our Hospital. As the name suggests, now getting rid of glasses is going to be as smooth as SILK.

1 - SILK – Smooth Incision Lenticule Keratomileusis :

SILK is the newest laser vision correction technology in the World. Based on the principle of corneal lenticule extraction. It’s performed by the ELITA Femtosecond Laser Platform released by Johnson &Johnson Vision.

While older platforms like SMILE -2012 and CLEAR - 2019 utilize similar processes as the ELITA Fem to second platform for lenticule extraction, the SILK procedure represents a significant advancement over these methods, establishing itself as the next-generation technology in correcting refractive errors.

How does SILK work?

SILK stands for Smooth Incision Lenticule Keratomileusis. It is an innovative eye surgery technique that utilizes a very advanced femtosecond laser to perform the smooth lenticule extraction. The procedure is based on lenticule extraction, making it flap-less, blade-free, and promoting quick recovery. The ELITA laser helps correct vision issues like myopia and astigmatism.

SILK works through the following steps:

  • Create : SILK utilizes the advanced ELITA laser eye technology to meticulously create a small disc-shaped lens(lenticule) within the cornea. This process is entirely flap-less and performed without the use of blades.
  • Remove: The created lenticule is then delicately extracted through a microscopic incision made by the laser. This step is crucial in adjusting the shape and curvature of the cornea to correct refractive errors.
  • Reshape: Once the smooth lenticule is removed, the cornea undergoes reshaping. This reshaping facilitates improved vision clarity for the patient, addressing issues such as myopia and astigmatism.

Ideal Candidate for SILK eye surgery include :

  • Age : Patients must be at least 18 years old.
  • Refractive Error : Myopia (nearsightedness) up to -12.00 D with and without astigmatism up to -6.00 D.
  • Stable Refractive Error : The patient should have had stable refractive error for at least 6 months.
  • Pregnancy and Birth Control : Patients should not be pregnant, lactating, or of child-bearing potential unless they are using a medically approved method of birth control.
  • No Previous Eye Surgery : No history of intraocular or corneal surgery of any kind. These criteria help ensure the safety and effectiveness of SILK surgery for eligible patients.

Advantages of SILK Surgery?

  • Flapless Procedure: SILK surgery is minimally invasive because it does not require creating a corneal flap like traditional LASIK. Instead, it forms a thin lenticule within the cornea, leading to faster recovery and less discomfort. It also prevents flap related complcations. Approximately 99% of patients are highly satisfied with this approach.
  • Enhanced Comfort: The unique biconvex shape of the lenticule reduces cutting of corneal nerves and provides faster nerve regeneration for a healthy ocular surface. It also has a positive impact on the long-term quality of vision. Additionally, it uses an ultra-precise pulse energy laser to smoothen the corneal surface. This reduces the risk of excessive corneal tissue damage due to laser, which could otherwise lead to some dry eyes. 90% of patients experience no or only minimal discomfort during and after the procedure.
  • Reduced Risk of Complication : By preserving the cornea’s strength, SILK surgery significantly lowers the risk of complications such as cornealectasia compared to other vision correction methods.
  • Quick Recovery: A high percentage of patients (99%)undergoing SILK laser vision correction experience rapid recovery, with improved vision typically within hours to a couple of days after the procedure, , allowing them to resume daily activities sooner.
  • Precision and Accuracy: SILK surgery utilizes advanced femtosecond laser technology to reshape the cornea with exceptional precision and accuracy. The procedure is highly customizable, catering to each patient's unique needs.
  • Enhanced Safety: It’s safer than LASIK for sportspersons, those in active combat or those in professions where there are chances of injury. This is because we do not make a corneal flap- which means the cornea remains more stable.

CONCLUSION

SILK stands as a beacon of modern medical innovation by deliveringa bladeless, flapless and painless experience. SILK has shown high patient satisfaction rates due to its advanced technology, minimal invasiveness, and ability to provide clear vision with reduced dependency on glasses or contact lenses.

SILK Laser Eye Surgery Procedure :

The refractive surge on thoroughly examines the patient's eyes to assess their suitability for laser vision correction, specifically ELITA SILK eye surgery. This involves: Checking the patient's vision prescription and overall eye health. Performing tests like corneal topography and pachymetry to measure corneal shape and thickness.

Conducting advanced tests to evaluate the retina. These tests help determine if the patient is a suitable candidate for ELITA SILK surgery based on their eye condition and overall health.

On the day of the laser eye surgery procedure, after the patient consents, they are taken to the Operation Theatre. An aesthetic eye drops are administered for comfort. ELITA's advanced laser system emits precise femtosecond pulses to create a biconvex lenticule in the cornea. The surgeon carefully dissects and gently extracts the lenticule through a microscopic incision typically 2-3 mm in size. This process reshapes the cornea, correcting the patient's vision.

After the procedure, patients typically go home within 30 minutes, though it may require total 2-3 hrs time to stay at laser center. They receive personalized counseling on post-operative medications and precautions. This includes prescribed eye drops for infection prevention and healing. Follow-up appointments are scheduled for the day after surgery later to monitor recovery and ensure optimal vision correction outcomes. These check-ups are essential for tracking eye health progress and maximizing results.

2 - Bladeless Contoura Lasik or Femto Lasik

Bladeless LASIK surgery, also known as Femtosecond LASIK or all-laser LASIK, represents a significant advancement invision correction procedures compared to traditional blade LASIK (Microkeratome surgery). Here's a comprehensive overview of its procedure, benefits, potential side effects, and conclusions.

Procedure of Bladeless LASIK Surgery :

  • Creation of the Flap : The procedure starts with the creation of a thin corneal flap using a Femtosecond laser. This laser opera test an ultra-fast speed , allowing precise flap creation without the use of a blade. This reduces the risk of complications associated with manual blade creation.
  • Lifting the Flap : Once the flap is created, it is gently lifted to access the underlying corneal tissue.
  • Reshaping the Cornea : An excimer laser is then used to reshape the cornea based on the patient's refractive error, correcting myopia, hyperopia, or astigmatism by removing precise amounts of cornea lt issue.
  • Repositioning the Flap: After the cornea is reshaped, the flap is carefully repositioned. The precision of the Fem to second laser ensures that the flap fits back seamlessly, minimizing the risk of complications such as debris or irregular healing.

Differences from Blade LASIK :

Bladeless LASIK differs primarily in the method of creating the corneal flap. Instead of a microkeratome blade, which is used in traditional LASIK procedures, the Femtosecond laser provides greater accuracy and safety during flap creation.!

Advantages of Bladeless LASIK Surgery :

  • Reduced Risk of Flap Complications: Since the procedure eliminates the variability associated with a blade, the risk off lap-related complications such as irregular flaps, buttonholes, or infections is significantly reduced.
  • More Precise Outcomes: The use of advanced lasers ensures more accurate correction of vision errors, leading to improved visual outcomes.
  • Less Post-Operative Discomfort: Patients often experience less discomfort and faster recovery times due to the precise nature of the procedure.
  • Lower Incidence of Dry Eyes: Compared to blade LASIK, bladeless LASIK has been associated with a lower risk of post-operative dry eyes.

Potential Side Effects :

While bladeless LASIK is generally safe, like any surgical procedure, it can have potential side effects:

  • Dislocated Flap: Rare instances of the corneal flap not properly adhering may occur.
  • Epithelial In-growth: Occasionally, cells from the outer layer of the cornea may grow under the flap, which can affect vision.
  • Halo and Glare: Some patients may experience temporary halo or glare around lights, especially at night, which typically improves over time.

Femto LASIK Eye Surgery

Conclusion :

Bladeless LASIK represents a highly effective and safe option for vision correction, offering superior precision and reduced risks compared to traditional blade LASIK. While complications are rare, it's essential for prospective patients to consult with experienced eye surgeons to determine if they are suitable candidates for the procedure.

Choosing a reputable eye hospital with skilled professionals and state-of-the-art technology, like the Shraddha Eye Hospital & Laser Centre mentioned, can enhance the likelihood of a successful outcome. Always ensure to discuss potential risks and benefits thoroughly with your eye care provider before proceeding with bladeless LASIK surgery.

3 - Contoura Lasik

In 1999, doctors developed wave front analysis, which maps the errors of a patient’s eye as a whole. The data is programmed into the laser, resulting in a customized procedure with much better results. Contoura Vision builds on the principle of topography, providing a precise mapping of 22,000 points of the cornea. Traditional LASIK wave-front only maps approximately 200 points of the entire eye.

SHARPEST VISION BEYOND LASIK AND SMILE

Contoura Vision is the latest advancement in Specs Removal by Laser Vision Correction. While procedures like LASIK and SMILE (Small Incision Lenticule Extraction)only correct specs power, Contoura Vision goes one step further!
